Abstract
Meme kanseri cerrahisinin tarihsel evrimi, antik medeniyetlerin bazı küçük cerrahi teknikler ve koterizasyon kullanmalarıyla başlamıştır. Meme kanseri cerrahisi, antik çağlardan bu yana dört dönemden geçmiştir. Bu dört dönem sırasıyla; Antik Dönem, Orta Çağ ve Rönesans Dönemi, 18. ve 19. yüzyıl ve 20. yüzyıl ve sonrası olarak bilinmektedir. Basit cerrahi müdahaleler ve koterizasyon gibi temel cerrahi teknikler kullanımından, William Stewart Halsted (1852-1922) tarafından tanımlanan radikal mastektomi gibi daha kapsamlı cerrahi prosedürlere ve radikal cerrahi yöntemler yerine meme koruyucu cerrahiye kadar büyük değişimler yaşanmıştır. Modern meme kanseri cerrahisinde dönüm noktası olarak bilinen en büyük çalışma NSABP B-04 olarak bilinmektedir. Bu çalışmayla birlikte mastektominin pektoral kasların çıkarılmadan güvenli bir şekilde yapılabeleceği kanıtlanmıştır. Diğer dönüm noktası ise meme koruycu cerrahinin kabul edilmesinde rolü olan NSABP-06 ve Milan çalışmalarıdır. Bu iki çalışmayla birlikte, mastektominin, meme koruyucu cerrahiye radyoterapi eklendiğinde hastalıksız ve genel sağkalım arasında herhangi bir fark gözlenmemiş olmasıdır. Son yıllarda ise estetik görünümün de ön plana çıkmasıyla, onkolojik prensipler ile plastik cerrahi tekniklerinin birleştiği onkoplastik cerrahi günlük pratik de yerini almaya başlamıştır.
The historical evolution of breast cancer surgery began with the use of some basic surgical techniques and cauterization by ancient civilizations. Breast cancer surgery has undergone four periods since ancient times. These four periods are known as the Ancient Period, the Middle Ages and the Renaissance Period, the 18th and 19th centuries, and the 20th century and beyond. Significant changes have occurred, progressing from simple surgical interventions and basic techniques like cauterization to more comprehensive surgical procedures such as the radical mastectomy defined by William Stewart Halsted (1852-1922), and ultimately to breast-conserving surgery instead of radical surgical methods. A significant turning point in modern breast cancer surgery is identified with the landmark study NSABP B-04. This study demonstrated that mastectomy could be safely performed without removing the pectoral muscles. Another pivotal moment is attributed to the NSABP-06 and Milan studies, which played a role in the acceptance of breast-conserving surgery. With these two studies, it was observed that when radiotherapy was added to breast-conserving surgery, there was no significant difference in disease-free and overall survival compared to mastectomy. In recent years, with an increasing emphasis on aesthetic appearance, oncoplastic surgery has started taking place in daily practice, combining oncological principles with plastic surgery techniques.
