    Survival outcomes of patients with oligometastatic non-small cell lung cancer who were treated with radical therapy: a multicenter analysis

    Background/aim: Oligometastatic disease for nonsmall cell lung cancer (NSCLC) patients is generally thought to represent a better prognosis with a quieter biology, limited number of disease sites and long-term disease control. In this study, we aimed to determine the efficacy of radical treatment options for patients with oligometastatic NSCLC. Materials and methods: This retrospective trial included totally 134 patients with oligometastatic NSCLC. The presence of oncodriver mutation, tumor stages and nodal status, the number of metastases and involved metastatic site, treatment of primary tumor and oligometastasis, response rate, overall survival (OS) and progression-free survival (PFS) were evaluated. Results: Of 134 patients 66.4% were defined as adenocarcinoma, 26.1% were squamous cell carcinoma and 7.5% of patients were in other histology. Based on the treatment of primary tumor, in 36 patients (26.9%) curative surgery has undergone, in addition, 19 (14.2%) patients were received chemotherapy, 73 (54.5%) were treated with chemoradiotherapy, while immunotherapy and targeted therapy were used in 1 (0.7%) and 2 (1.4%), respectively. The preferred treatment for oligometastatic lesions were SBRT in 72.4% of patients, surgery in 10.5%, and both SBRT and surgery in 17.1% of patients. At the median follow up of 31.3 months (range: 9.5–48.5), the median PFS and OS times were 17 and 24.4 months, respectively. Moreover, OS-2 after progression was also 7.2 months. Conclusion: Based on our real-life experience, we demonstrated a significant correlation between good response to first treatment and survival in oligometastatic disease, we also understand that local ablative treatment modalities prolong and also delay both OS and PFS in oligometastatic NSCLC patients OS-2. © TÜBİTAK.

    Negotiorum gestio as a source of obligation: from Roman law to modern codes

    The legal institution of Negotiorum Gestio refers first and foremost to the act of helping or aiding someone in need, typically with the intention of doing good or promoting the well-being of the recipient. This concept has a long history, with roots in Roman law and its diversified influence on modern legal systems. In this article, we explore the evolution of negotiorum gestio from its origins in Roman law to its current manifestation in modern civil codes while providing an examination of how the concept has been defined, understood, and applied within Roman law over time as well as of its long journey through out ius commune to the modern codification era. Being a strictly Roman law institution, the prevalent incorporation of negotiorum gestio into the codes of the ‘civil law’ jurisdiction as well as its designation as one of the sources for non-contractual obligations under the harmonized rules of ‘EU Rome II Regulation’ calls for a close-in analysis of this originally Roman concept which will shed light on the degrees of the evolution, transformation and reception it had experienced while helping to make sense of its current state in modern civil law. © 2023 Authors.     Comparison of the dosimetric planning efficiency of dynamic conformal arc and volumetric modulated arc therapy techniques for stereotactic body radiotherapy of lung cancer using ınternal target volume

    Objective: This retrospective study constitutes a feasibility assessment of dynamic conformal arc (DCA) therapy as an alter-native to volumetric-modulated arc therapy (VMAT) for stereotactic body radiation therapy (SBRT) of lung cancer (LC) with the free breathing technique using internal target volume. DCA is to create a more efficient treatment reducing beam-on time (BOT) and monitor unit (MU) without interplay errors except for complex tumor geometries when compared to volumetric modulated arc therapy (VMAT).Materials and Methods: CT images and plans of forty patients treated with 50 Gy prescription in four fractions using VMAT technique for SBRT treatment of LC selected. Plans were re-planned with using DCA technique. VMAT and DCA plans compared via The Radiation Therapy Oncology Group (RTOG) Protocol 0915 for conformity and efficient having pass rate of gamma index in quality assurance (QA), MU and BOT.Results: The study included 40 patients. The mean value of QA pass rate 99.10 +/- 1.49 in DCA and 92.34 +/- 1.96 in VMAT. The rate was higher in DCA (p<0.001 and t=8.98). The values of BOT and MU were 4.68 min and 3296 in the VMAT tech-nique and they were 3.58 min and 2395 in DCA. These values were significantly improved with DCA (p<0.001 and p<0.001).Conclusion: DCA can potentially minimize multi-leaf collimator errors from respiratory motion and small-field dosimetry. It delivery similar doses of treatment quality to tumor while providing faster treatment by significantly reducing MU and BOT compared to VMAT and moreover offers same-day SBRT treatments without the need for specific QA.

    Status of pediatric oncology nurses to achieve global standards: the sample of Türkiye

    Objective: This study’s purpose is to investigate the achievement of baseline standards for pediatric oncology nursing care in Türkiye. Materials and Methods: The research was carried out with nurse administrators in 41 pediatric oncology clinics between March and May 2022. Data were collected by an 'Information Form' and ‘Baseline Standards for Pediatric Oncology Nursing Care’. Results: Most of the clinics did not meet the first standard related to staffing based on patient acuity. It was found that 80.5% of the clinics that applied formalized education in pediatric oncology clinics, and they met the second standard related to formalized orientation. Most of the clinics (92.7%) had continuing education and training and they met the third standard. Most of the clinics (82.9%) met the fourth standard related to multidisciplinary teamwork. Nearly half of the clinics met the fifth and sixth standards. Conclusion: It was revealed that there are not many specialist nurses working in the clinics, that the nurse-to-patient ratio is relatively high, and nurses do not receive sufficient financial support for research. Institutions and societies must create strategies that specify pediatric oncology nurses to the achievement of baseline standards.

    The effects of psychosocial interventions on the caregivers of pediatric oncology patients on psychological symptoms: a systematic review, Turkey sample

    Amaç: Bu çalışmada Türkiye’de pediatrik onkoloji hastalarının bakım verenlerine uygulanan psikososyal girişimlerin ruhsal semptomlara olan etkisini inceleyen araştırmaların incelenmesi hedeflenmiştir. Yöntem: Deneysel ve yarı deneysel araştırmalar PRISMA protokolü doğrultusunda dokuz veri tabanında; (Pubmed, Cochrane, MEDLINE, CINAHL, EMBASE, Google Akademik, Web of Science, Ulusal Tez Merkezi, ULAKBİM), Ocak 2010-2021 tarihleri arasında başlık ve özete göre tarandı. ‘Kanserli çocuk’ veya ‘pediatrik onkoloji, ‘psikososyal uygulamalar’, ‘hemşirelik’, ‘ruhsal semptomlar’, ‘bakım verenler’ veya ‘ebeveynler’ anahtar sözcükleri Türkçe ve İngilizce olarak kullanılarak veri tabanlarında arama yapıldı. Referans yönetimi için Endnote X9 kullanıldı. Araştırmaların kalite indeksi Joanna Briggs Enstitüsü’nün kontrol listeleri kullanılarak değerlendirildi. Bulgular: Derlemeye beş araştırma dahil edildi. Dahil edilen araştırmaların üçü doktora tezi, ikisi ise bireysel araştırmadır. Psikososyal girişim olarak teori temelli hemşirelik uygulaması, psikoeğitim programı, tedavi ve hastalığa uyuma yönelik eğitimlerin verildiği belirlendi. Psikososyal girişimlerin, bakım verenlerin hastalığa yönelik belirsizlik ve umutsuzluk algısı, kaygı düzeyi, bakım yükü, depresyon ve ruhsal sorunlarını azalttığı; yaşam doyumu ve stresle baş etme becerilerini arttırdığı belirlendi. Sonuç: Pediatrik onkoloji hastalarının bakım verenlerine uygulanan psikososyal girişimlerin bakım verenlerin ruhsal sorunlarını azaltma ve iyilik hallerini arttırmada olumlu etkiye sahip olduğu belirlendi. Bu müdahalelerin etkinliğini iyileştirmek ve potansiyel etkilerini yaygınlaştırmak amacıyla daha fazla çalışmaların yürütülmesi önerilmektedir.

    A compendious review of majorization-based resource theories: quantum information and quantum thermodynamics

    The field of quantum resource theory (QRT) has emerged as an invaluable framework for the examination of small and strongly correlated quantum systems, surpassing the boundaries imposed by traditional statistical treatments. The fundamental objective of general QRTs is to characterize these systems by precisely quantifying the level of control attainable to an experimenter. In this review article, we refrain from providing an exhaustive summary of the extensive literature on QRT. Rather, our focus centers on a specific subliterature founded upon the theory of majorization. The primary aim is to augment our comprehension of genuine quantum phenomena manifested across diverse technological applications and incite investigations into novel resource theories encompassing multiple types of resources. Consequently, we emphasize the underlying similarities shared by various resources, including bipartite quantum entanglement, quantum coherence, and superposition, alongside informational, thermal, and generalized nonequilibrium resources. © 2023 TUBITAK.     A 57-year-old female patient presented with fever, nausea, vomiting, loss of appetite, and weight loss within the last two months. Ceftriaxone and metronidazole therapy was start-ed upon discovery of a liver abscess but provided no benefit. Following the of abscess bi-opsy, the patient developed fever, itching, anemia, acute renal failure, hyperbilirubinemia, and eosinophilia that required intensive care unit (ICU) admission. The Fasciola hepatica antibodies were detected by enzyme-linked immunosorbent assay (ELISA). Triclabendazole was started, after which the symptoms and magnetic resonance imaging (MRI) findings regressed. Even without eosinophilia, F. hepatica should be considered in cases with a liver abscess that does not respond to antibiotics.

    Obstructive sleep apnea and cardiovascular disease: where do we stand?

    Obstructive sleep apnea is common in adults with cardiovascular disease. Accumulating evidence suggests an association between obstructive sleep apnea and cardiovascular disease independent of the traditionally recognized cardiovascular disease risk factors. Observational studies indicate that obstructive sleep apnea is a risk factor for development of cardiovascular disease and that alleviation of obstructive events with positive airway pressure may improve cardiovascular disease outcomes. However, recent randomized controlled trials have not supported the beneficial effect of positive airway pressure in cardiac populations with concomitant obstructive sleep apnea. Some evidence suggests that the relationship between obstructive sleep apnea and traditionally recognized cardiovascular disease risk factors is bidirectional, suggesting that patients with cardiovascular disease may also develop obstructive sleep apnea and that efficient treatment of cardiovascular disease may improve obstructive sleep apnea. Recent data also indicate that the apnea-hypopnea index, which is commonly used as a diagnostic measure of obstructive sleep apnea severity, has limited value as a prognostic measure for cardiovascular disease outcomes. Novel markers of obstructive sleep apnea -associated hypoxic burden and cardiac autonomic response seem to be strong predictors of adverse cardiovascular disease outcomes and response to treatment of obstructive sleep apnea. This narrative review and position paper from the Turkish Collaboration of Sleep Apnea Cardiovascular Trialists aims to update the current evidence about the relationship between obstructive sleep apnea and cardiovascular disease and, consequently, raise awareness for health professionals who deal with cardiovascular and respiratory diseases to improve the ability to direct resources at patients most likely to benefit from treatment of obstructive sleep apnea and optimize treatment of the coexisting cardiovascular diseases. Moreover, the Turkish Collaboration of Sleep Apnea Cardiovascular Trialists aims to contribute to strengthening the efforts of the International Collaboration of Sleep Apnea Cardiovascular Trialists in this context.

    Estimation of genetic correlation between rheumatoid arthritis and multiple sclerosis using summary statistics from genome-wide association studies

    Objective: Genome-wide association studies (GWAS) have revolutionized our understanding of the genetic basis of diseases by examining millions of genetic variants across the genome. Rheumatoid arthritis (RA) and multiple sclerosis (MS) are chronic autoimmune diseases characterized by immune system dysregulation and inflammation. Investigating the genetic correlation between RA and MS can provide insights into shared genetic factors, potential mechanisms, and pathways underlying these complex disorders. The objective of this study was to compare different statistical methods to estimate the genetic correlation between RA and MS using GWAS summary statistics.Materials and Methods: To estimate single nucleotide polymorphism (SNP) heritability and genetic correlation, we utilized two popular methods: Linkage Disequilibrium Score Regression (LDSC) and Linkage Disequilibrium Adjusted Kinship (LDAK) models.Results: Our analysis revealed a significant, moderate, positive correlation between RA and MS using both LDSC and LDAK (LSDCMS-RA=0.448, LDAK(MS-RA)=0.387, Spearman(MS-RA)=0.0262, p<0.001). Additionally, there were notable differences in heritability estimates between the two methods and the traits. The LDAK model demonstrated higher heritability estimates for the RA-MS relationship (h(MS-RA)(2) =0.314) compared to the LDSC (h(RA-MS )(2)=0.138).Conclusion: There is a significant positive genetic correlation between RA and MS, indicating a shared genetic component. Differential heritability estimates from LDAK and LDSC highlight the importance of the method. Genetic overlap informs common pathways and potential therapeutic targets. These findings contribute to the evidence of a moderately positive genetic correlation, emphasizing the need for further research and personalized approaches to managing autoimmune diseases.

    Investigation of substantia nıgra hyperechogenicity by transcranial sonography in patients with essential tremor

    Objective: Essential tremor (ET) is the most common movement disorder. ET diagnosis may precede future Parkinson’s disease. Substantia nigra hyperechogenicity in transcranial sonography (TCS) is associated with Parkinson’s disease. The underlying etiology of substantia nigra hyperechogenicity remains unclear. In this study, we aimed to investigate the significance of the substantia nigra hyperechogenicity in patients with essential tremor. Methods: A total of 55 patients with ET and 60 matched controls underwent TCS. The hyperechogenic area was measured in the SN. Approximately ten years after their baseline TCS, all patients were inquired about their current condition, treatment, and medications to determine whether they had received a diagnosis of PD. Results: A total of 15 subjects were excluded due to insufficient image acquisition. The echogenic area of the SN ranged from 0.01 to 0.86 cm2 (mean 0.25±0.15) in the patient group and from 0.02 to 0.72 cm2 (mean 0.15±0.16) in the control group. Patients with ET had a significantly larger echogenic area than controls (p=0.001). 47% (26/55) of the patients have hyperechogenic SN, whereas only 15 % (9/60) of healthy controls have hyperechogenicity in the substantia nigra. None of the patients reported having a change in their diagnoses of ET. Conclusions: The results of this study show that SN hyperechogenicity is increased in patients with ET. Transcranial sonography may contribute to understanding the pathophysiology of ET.
