    Approaches of pediatric nephrologists to hypertensive patients in Turkey (Turkish pediatric hypertension working group study)

    Objective: We aimed to evaluate the approaches of pediatric nephrologists in our country to the management of childhood hypertension. Methods: The pediatric nephrologists in our country were invited to fill out an online questionnaire including 24 questions. The answers were compared between those working in the field for <= 10 years (Group 1, n =74) and >10 years (Group 2, n = 62). Results: of 136 participants (M/F = 101/35), 52% were following a single guideline [31% Fourth Report of 2004, 17% European Society of Hypertension in 2016, and 52% American Academy of Pediatrics in 2017], which is more common in Group 1 (P =.035). The most commonly used guideline was American Academy of Pediatrics of 2017 and Group 2 used Fourth Report of 2004 more commonly (P =.042). The most common choice to diagnose hypertension was office + home + ambulatory blood pressure monitoring (59%). The frequency of screening for end-organ damage at first evaluation was 96%. The time to wait for the effect of lifestyle modifications was 3 months in 52%. The first choice medication was angiotensin-converting enzyme inhibitors (49%) or calcium-channel blockers (48%) in non-obese and angiotensin-converting enzyme inhibitors (74%) in obese children. Calcium-channel blockers were more commonly prescribed as the first choice in non-obese children in Group 1 (P =.035). The most accessible emergency drug was esmolol. Conclusion: Despite following recent guidelines, the time spent in the proficiency would change the practices.

    Association between elevated serum uric acid and vitamin D insufficiency among the middle-aged and elderly population

    OBJECTIVE: Vitamin D insufficiency might have a role in numerous diseases including autoimmune disease, cancer, diabetes mellitus, hypertension and heart diseases. The relationship between vitamin D insufficiency and hyperuricemia has been shown previously but there are conflicting results in studies. MATERIAL and METHODS: A total of 1562 patients who had serum uric acid and vitamin D levels measured at the same time were enrolled. Patients who were on vitamin D replacement therapy, receiving calcium and/or allopurinol, or had gout and chronic kidney disease were excluded. RESULTS: Hyperuricemic patients had significantly lower levels of serum vitamin D level compared with normouricemic patients (p<0.001) whereas there was no difference between the groups in terms of serum calcium, phosphorus, parathormone and alkaline phosphatase. Severe deficiency (25(OH) vitamin D <10) was significantly more common among patients with hyperuricemia (p<0.001). When vitamin D levels were analyzed according to age, a significant inverse correlation between vitamin D and serum uric acid level was found in decades 7 and 8. Age, eGFR and vitamin D level below 20 appeared as independent associates of serum uric acid levels. CONCLUSION: These data suggest that hyperuricemia associates with vitamin D deficiency. Further studies are needed to understand the mechanism underlying this association and its potential clinical implications. / AMAÇ: D vitamini eksikliği başta otoimmünite, kanser, diyabetes mellitus, hipertansiyon ve kalp hastalıkları olmak üzere birçok hastalıkla ilişkili olabilir. D vitamini eksikliği ile hiperürisemi arasında çelişkili sonuçlar daha önceki çalışmalarda gösterilmiştir. GEREÇ ve YÖNTEMLER: Serum D vitamini ve ürik asit değerleri eş zamanlı bakılan hastalar çalışmaya dahil edildi. D vitamini replasman tedavisi alanlar, kalsiyum ve/veya allopurinol kullananlar, gut ve kronik böbrek yetmezliği (glomeruler filtrasyon hızı <60 ml/min) olan hastalar çalışmaya dahil edilmedi. BULGULAR: Hiperürisemik hastaların serum vitamin D düzeyleri normoürisemik hastalara göre daha düşük olduğu görülmesine (p<0.001) karşın, gruplar serum kalsiyum, fosfor, parathormon ve alkalen fosfataz düzeyleri bakımından benzerdi. D vitamini düzeylerine göre değerlendirildiğinde ağır (vitamin D <10) düzeyde eksikliği olan hastaların daha çok hiperürisemik (p<0.001) grupta olduğu görüldü. Yaşa göre serum D vitamini ve ürik asit düzeyleri arasında anlamlı derecede negatif korelasyonun 7.ve 8. dekatlarda olduğu görüldü. Yaş, serum D vitamini düzeyinin <20 olması ve eGFR düzeyleri, serumürik asit düzeyi ile anlamlı korelasyon gösterdiği görüldü. SONUÇ: Çalışmamızda, hiperüriseminin D vitamini eksikliği ile ilişkili olduğu saptanmıştır. Bu ilişkiyi açıklayabilecek mekanizma ve bunun klinik açıdan etkilerine yönelik daha ileri çalışmalara ihtiyaç vardır.

    First virtual uro-oncology meeting during the COVID-19 pandemic: 10th Online Eurasian Uro-oncology Congress

    covid-19 has rapidly spread and has become a pandemic by affecting the whole world. During this period, many scientific congresses and educational meetings had to be canceled because of preventive measures. In this report, we aimed to share our first live virtual congress experience, described its process of transformation from face to face to virtual congress and report the attendees and speakers’ satisfaction. Material and Methods: Eurasian Uro-oncological Association (EUA) decided to organize the 10th Eurasian Uro-oncology congress in June 2020 at Göbeklitepe, Şanlıurfa in Turkey. However, due to the covid-19 pandemic, the organizing committee decided to organize the first virtual scientific congress in Turkey. The planned duration of the congress was reduced from 4 days to 2 days and each speaker was planned to give the speech online during the presentation via ZOOM program (San Jose, CA). Results: A total of 704 persons registered to the congress. It was the highest number of participants among whole congresses that was organized by EUA. In this congress, there were 199 oral presentations, 25 interactive e-posters and 12 video presentations. During the congress, each participant attended the congress for an average of 387 minutes. It was identified that the majority of the participants were quite satisfied with the program offered. In general, participants were fairly satisfied with the quality of images and sound, chat functionality, questions & answers section and technical support. Conclusion: In this report, we shared the outcomes of our first virtual congress experience in Turkey through the 10th Eurasian Uro-oncology Congress. Today, virtual congresses have become the “new normal” and offer cheaper events with larger participation in the comfort of home. / Öz: COVİD-19 hızlıca yayılarak kısa sürede tüm dünyayı etkileyen bir pandemi haline gelmiş ve bu süreçte koruyucu önlemler nedeniyle birçok bilimsel kongre ve eğitim toplantısı iptal edilmek zorunda kalınmıştır. Bu çalışmada, ilk canlı, online kongre deneyimimizi paylaşmayı, yüz yüze geleneksel kongreden online kongreye geçiş sürecini anlatmayı ve katılımcı ve konuşmacıların memnuniyetlerini ortaya koymayı amaçladık. Gereç ve Yöntemler: Avrasya Üroonkoloji Derneği (AÜD), 10. Avrasya Üroonkoloji Kongresi’ni Haziran 2020’de Şanlıurfa-Göbeklitepe, Türkiye’de düzenlemeye karar vermişti. Ancak organizasyon komitesi COVİD-19 pandemisi nedeniyle Türkiye’deki ilk online kongreyi düzenlemeye karar verdi. Planlanan kongre süresi 4 günden 2 güne düşürüldü ve her konuşmacının sunumlarını ZOOM programı (San Jose, CA) üzerinden yapmaları planlandı. Bulgular: Toplam 704 kişi kongreye kayıt yaptırdı. Bu sayı ile AÜD tarafından düzenlenen tüm kongreler arasında en fazla katılımcı sayısına sahip kongre bu kongre oldu. Kongrede 199 sözlü sunum, 25 interaktif e-poster ve 12 video sunumu yer aldı. Kongre süresince her katılımcı ortalama 387 dakika kongreye katıldı. Katılımcıların çoğunluğunun sunulan programdan oldukça memnun olduğu tespit edildi. Katılımcıların genel olarak görüntü ve ses kalitesinden, sohbet fonksiyonundan, soru-cevap bölümünden ve teknik destekten oldukça memnun olduğu saptandı. Sonuç: Bu makalede, Türkiye’deki ilk online kongre deneyimimizin sonuçlarını 10. Avrasya Üroonkoloji Kongresi ile sunduk. Günümüzde online kongreler “yeni normal” haline gelmiş durumdadır ve ev konforunda daha fazla katılımcı ile daha az maliyetli etkinlikler sunmaktadır.

    Effects of covid-19 lockdown on people's sexual lives in Turkiye

    Objective: To evaluate the impact of coronavirus disease-2019 (COVID-19) lockdown on the sexual lives of couples who live in Turkiye.Materials and Methods: One hundred ninety-three sexually active participants. While sexual functions were evaluated with the international erectile function index-15 in men and the female sexual function index in women, Hamilton anxiety scale (HAM) was used to measure anxiety level. Married people filled out the Marriage adjustment test (MAT) questionnaire. The effects of the quarantine period on the relationship were evaluated with a questionnaire created specifically for this study.Results: Sexual improvement and worsening were observed in 8.8%, and 15%, respectively. It did not change for 76.2%. Sexually deteriorated subjects had a higher HAM score (p=0.003). The MAT score was lower in sexually deteriorated subjects (p=0.004). The rate of sexual worsening was higher in women than in men (28.6% vs. 12%, p=0.02). Women's HAM scores were higher (p=0.002). The MAT score was also found to be higher in women (p=0.0037). 58% of sexually deteriorated participants did not feel safe at home during the COVID-19 period, whereas all of those sexually improved participants felt safe at home.Conclusion: During the COVID-19 lockdown period, sexual deterioration was detected in 15% of sexually active people. This worsening was associated with the HAM score. Additionally, it was revealed that feeling safe at home is related to sexual life. While the relationship between sexually worsening and the MAT score was significant in men, it was revealed that the level of anxiety in women affected sexual life more significantly.

    Warfarin-induced skin necrosis due to protein C deficiency in a dialysis patient

    Protein-C (PC) is a vitamin-K-dependent anticoagulant proenzyme produced by the liver. PC deficiency may cause both venous and arterial thromboses. In patients with PC deficiency, warfarin further decreases PC activity and causes thrombosis of skin arterioles leading to skin necrosis. A 59-year-old female was admitted with dyspnea, cough, hoarseness and edema in her neck and arms. She had chronic kidney disease for 20 years. She had been on hemodialysis for 8 years but had been switched to peritoneal dialysis due to vascular access problems caused by multiple venous thromboses. With a pre-diagnosis of Superior Vena Cava (SVC) syndrome, cavography was performed and near-total occlusion of the SVC was detected. Balloon dilatation was performed and warfarin 5 mg and enoxoparin 40 mg were started. Within a day, necrotic and well-demarcated lesions 4x5 cm in size appeared on the arm. Warfarin was stopped and enoxoparin was continued. After 2 weeks, plasma PC activity was found to be significantly low (40% of normal). The diagnosis of "warfarin-induced skin necrosis in a patient with PC deficiency" was established. Skin lesions promptly and completely recovered after the treatment. PC deficiency should be considered in dialysis patients with multiple thromboses, vascular access problems and warfarin-induced skin necrosis.

    Sarcomatoid carcinoma of the urinary bladder: a case report and review of the literature

    Sarcomatoid variant of urothelial carcinoma of the urinary bladder is a rare malignant neoplasm, which is defined with biphasic neoplasm with both an epithelial and a mesenchymal component by histopathological and immunohistochemistry. It is responsible for 0.3% off all bladder carcinomas. The aim of this study was to present the histopathological and immunohistochemical findings of a case of sarcomatoid carcinoma of the urinary bladder and to review the relevant literature. A 75-year-old man was presented to our hospital with hematuria. 6.5x5.8 cm tumor was found in the urinary bladder at ultrasonography. Magnetic resonance imaging demonstrated a large irregular-shaped 9x5 cm tumor involving the right posterolateral wall of the urinary bladder and an atrophic kidney. The patient underwent cystoprostatectomy, left nephrectomy, and lymph node dissection. Macroscopically, a 7.5x6.1x1.8 cm ulcero-vegetating tumor mass infiltrating the bladder base, right-left side wall, anterior-posterior wall, and trigon was seen. Microscopically, this tumor was composed of high-grade urotelial carcinoma and mesenchymal differentiation areas with a wide eosinophilic cytoplasm, vesicular nucleus, distinct nucleolus and spindle-shaped areas. Immunohistochemically, mesenchymal differentiation areas were stained prominently with vimentin, keratin. The case was diagnosed as sarcomatoid carcinoma. Tumor metastasis was detected in the prostate and in bilateral pelvic lymph nodes. Although the clinical findings and the age of diagnosis of sarcomatoid urothelial carcinoma are similiar to conventional urothelial carcinoma, it has a worse prognosis. The presented case died 2.5 months after surgery. Sarcomatoid carcinoma seems to represent the final common pathway of urothelial carcinoma dedifferentiation. Keeping this aggressive tumor type in mind is essential for a correct treatment approach.
