    Crohn's disease of the vulva: a unique presentation with an unconventional treatment approach

    Vulva's Crohn's disease (CD) is a rare condition that can be easily misdiagnosed if not thoroughly investigated. The typical presentation of the disease is swelling, deep linear fissures, erythema and painful, and progressive ulceration of the labia. There are various treatment protocols, yet none of them have been established as gold standard. Surgery is not recommended, and medical treatment is the current standard of care. We aim to report an unusual presentation of vulvar CD and discuss the treatment option of surgery, a valuable alternative when medical therapy yields ineffective results. A 36-year-old patient, whose initial symptoms had begun 7 years ago, presented with labial fistulas and multiple tissue papillomas. During the disease course, the patient had previously received corticosteroid and antibiotic treatments of limited benefit. Vulvar hypertrophy was significant, and papilloma formation related to the extension of the knife-like ulcers had left the outer genitals dysmorphic. The patient underwent two consecutive partial vulvectomies with 1 year apart, along with oral moxifloxacin treatment. Partial vulvectomies reduced the amount of hypertrophic tissues and papillomas and allowed for asymptomatic urine output. The patient also had psychological relief and experienced less intervening intertriginous fungal infections. Due to the ongoing inflammatory process of CD, symptomatic and clinical relief by stabilizing the condition is of utmost importance. Although medical treatment is frequently sufficient to stabilize symptoms of vulvar CD, surgery certainly reserves a place in the treatment algorithm when the patient is unresponsive to other treatment modalities.

    Should calcineurin inhibitors/sirolimus be ceased completely in posterior reversible encephalopathy syndrome?

    Background. To investigate the relationship between immunosuppressive treatments and posterior reversible encephalopathy syndrome (PRES) in transplant patients. Methods. We presented a retrospective study of 4 cases of PRES in transplant patients. Patient records were reviewed to identify potential risk factors, clinical presentations, radiological find- ings, and immunosuppressive treatments used. Results. Our analysis revealed a potential association between immunosuppressive treatments and the development of PRES in transplant patients. Specifically, we found that adjusting or switching immunosuppressive treatments can improve outcomes and prevent the recurrence of PRES. Conclusion. Our findings highlight the importance of recognizing PRES as a potential complication of immunosuppressive treatments in transplant patients. Early detection and management, including a review of immunosuppressive treatments, may improve patient outcomes and prevent further complications.

    Could DTI unlock the mystery of subjective tinnitus: it’s time for parameters that go a little out of the routine

    In this study, it was aimed to assess the microstructural changes in the main central auditory pathway in cases with subjective tinnitus. In total, 101 subjects (52 cases with bilateral subjective non-pulsatile tinnitus and 49 healthy cases as the control group) were included in the study. Participants underwent pure tone audiogram and Diffusion Tensor Imaging-Magnetic Resonance Imaging (DTI-MRI) examination with a 3 Tesla MRI device. The number of tracts, tract length, volume, and quantitative anisotropy (QA) and normalized quantitative anisotropy’ (nQA) values were calculated by plotting cochleocortical pathways from the cochlear nerve to ipsilateral and contralateral Heschl’s gyrus (HG). In pure tone audiometry, the control group had lower hearing thresholds than cases with tinnitus. Fibres and nQA values from the right cochlear nerve to the right HG were significantly lower in the tinnitus group than in the control group. Cochlear nuclei voxel counts were significantly decreased in the tinnitus group. Both cochlear nucleus volumes were higher in the tinnitus group than in the control group. nQA values in both cochlear nuclei were decreased in the tinnitus group. This study showed that the most commonly affected part in subjective non-pulsatile tinnitus cases is the cochlear nucleus. Therefore, the cochlear nucleus should be evaluated more carefully in cases presenting with subjective tinnitus.

    We read the study conducted by Joseph and colleagues with great interest, which investigated the loco-regional control, disease-specific survival (DSS), overall survival (OS), and treatment-related complications in 163 oral cancer (OC) patients treated with radiotherapy (RT) or chemo-RT (CRT) for close resection margins (CRMs).The study results offer valuable insights into the role of RT/CRT in OC patients with CRMs, but two concerns must be addressed to interpret the outcomes rigorously.

    Primary angiosarcoma of the pancreas - a case report and review of the literature

    BackgroundAngiosarcomas are malignant neoplasms that originate from endothelial cells.The symptoms exhibit a non-specific nature, and achieving a preoperative diagnosis is frequently challenging. They are seldom encountered in the abdomen, and their occurrence in the pancreas is even rarer.Methods Here we document a 67-year-old man with pancreatic angiosarcoma and analyse the literature to outline the clinicopathologic characteristics of this rare phenomenon.ResultsThis patient with family history of pancreas cancer presented with abdominal pain, and the CT-scan revealed a 4 cm mass at the neck of the pancreas but CA19-9 was normal. Radiologic findings were unusual for ordinary pancreas cancer. Fine-needle aspiration biopsy through endoscopic ultrasound revealed "undifferentiated malignant cells for which the diagnosis of "carcinoma" was favoured. Total pancreatectomy, splenectomy and portal vein reconstruction were performed and epithelioid angiosarcoma were diagnosed. Despite an uneventful postoperative period, discharge on postoperative day 8 without any complications, as well as diligent post-discharge clinical care, the patient died 65 days postoperatively, attributed to the presence of extensive metastasis. A comprehensive literature search has identified a limited number of documented cases of primary pancreatic angiosarcoma, with only ten cases reported to date.ConclusionsPancreatic angiosarcomas are very rare and prone to misdiagnosis. The formation of a more demarcated but high-grade tumour with necrosis is a feature that distinguishes angiosarcomas from ordinary carcinomas of this organ. Pathologic diagnosis is also highly challenging closely resembling undifferentiated carcinomas. Angiosarcomas are highly aggressive when they occur in the pancreas. Prompt diagnosis at an early stage is crucial as surgery with curative intent serves as the primary treatment approach. Surgery with curative intent is the mainstay treatment for pancreatic angiosarcoma when diagnosed at an early stage.Oncological treatment options should be taken into consideration according to the follow-up data.Why does this paper matter?This article is important in that it is the most comprehensive review of the literature on pancreatic angiosarcoma, which is a very rare pathology, from the perspective of radiology, pathology and surgery.

    Mesotherapy is widely used for its lipolytic effect as an alternative procedure to surgical methods. Although many benefits of lipolytic mesotherapy have been observed, numerous side effects have also been reported. Here, we report a case of cutaneous foreign body granulomas that occurred after lipolytic rnesotherapy.

    Clinical results of anterior odontoid screw fixation for type II odontoid fractures

    AIM: In this study, 31 patients with a diagnosis of Type II odontoid fractures were reported. All patients were treated with anterior transodontoid screw fixation and clinical outcomes were reported. MATERIAL and METHODS: In this study, the retrospective clinical analysis of 31 patients with traumatic type II odontoid fractures who were treated through anterior transodontoid fixation in Neurosurgery Department at VKV American Hospital between 1998 and 2012 was performed. The age, sex, cause of injury, diagnosis time, neurological examination before and after surgery, follow-up period were evaluated. The neurological status of patients was classified according to the Frankel scale. RESULTS: In 4 patients, 2 transodontoid screws were inserted. The mean hospital stay was 3.35 days. Posterior occipito-cervical fusion was done in 1 patient due to the lack of fusion in the first operation. No vascular injury, screw malposition, infection, neurologic deterioration, or complications were observed during the peroperative and postoperative stage. The mean postoperative follow-up period was 36 months after surgery. Radiological imaging of patients were performed at the early and late postoperative stage. CONCLUSION: We found satisfying fusion rates and better patient comfort during the postoperative period. We think that stabilization and fusion through a transodontoid screw is a minimal invasive method.AMAÇ: Çalışmada, Tip II odontoid fraktürü tanısı alan 31 olgu bildirilmiştir. Olguların hepsi anterior transodontoid vida fiksasyonu ile tedavi edilmiş ve klinik sonuçları bildirilmiştir. YÖNTEM ve GEREÇLER: Çalışma, VKV Amerikan Hastanesi Nöroşirürji Bölümü’nde 1998-2012 yılları arasında travmatik tip II odontoid kırığı nedeniyle anterior transodontoid fiksasyon ile cerrahi tedavi uygulanan 31 hastanın retrospektif klinik analizleri yapılmıştır. Tüm olguların yaş, cinsiyet, travma nedeni, teşhis zamanı, cerrahi girişim öncesi ve sonrasında nörolojik muayeneleri, takip süresi değerlendirilmiştir. Olguların nörolojik durumları Frankel skalasına göre sınıflandırılmıştır. BULGULAR: Dört hastaya 2 adet transodontoid vidası yerleştirildi. Hastanede kalış süresi ortalama 3,35 gün idi. 1 hastada postop takibinde füzyon oluşmaması nedeniyle ikinci bir operasyon ile posteriordan oksipitoservikal füzyon yapıldı. İşlem sırasında ve sonrasında damar yaralanması, vida malpozisyonu, enfeksiyon, nörolojik bozulma gibi komplikasyonlar izlenmedi. Hastalar ameliyat sonrasında ortalama 36 ay takip edildi. Hastaların erken ve geç dönemde radyolojik görüntülemeleri yapıldı. SONUÇ: Postoperatif dönemde hastaların takiplerinde tatmin edici füzyon oranı ve hasta konforunun daha iyi olduğunu saptadık. Tip II odontoid kırıklarının cerrahi tedavisinde minimal invaziv yöntem olan transodontoid vida ile fiksasyonun yüksek avantajlarından dolayı ilk seçenek olabileceğini düşünüyoruz.
