This epidemic demonstrated the importance for healthcare systems and nurses worldwide to be adaptable and flexible once again. The Turkish Nurses Association acted at the national and international level and actively participated in the process, starting with the announcement of the first case, in order to overcome the pandemic with minimal damage. These activities included sharing scientific and updated information for healthcare professionals and the public; informing the Ministry of Health and related units and preparing reports on the suggested solutions to these problems and the problems experienced by nurses; and preparing and sharing care algorithms and videos. In our country, nurses have performed their professions in the best way possible under difficult conditions in order to maintain safe and qualified nursing care at the highest level. However, necessary initiatives and investments should be made urgently to include more nurses in leadership positions in order for them to contribute to the policy-making and decision-making processes. /Öz: Dünya Sağlık Örgütü’nün “pandemi” olarak kabul ettiği COVID-19 salgını, içinde bulunduğumuz küreselleşme çağında, insanlığın karşı karşıya kaldığı ciddi bir afettir. Bu salgın, ülkelerin sağlık sistemlerinin, çevik, uyarlanabilir ve esnek yapıda olmasının önemi ve hemşirelerin kilit konumunu bir kez daha ortaya koymuştur. Türk Hemşireler Derneği, pandemi sürecinin en az zararla atlatılması için, ilk olgunun açıklanmasından başlayarak ulusal ve uluslararası düzeyde faaliyetlerde bulunmuş ve sürecin içinde aktif bir şekilde yer almıştır. Bu faaliyetler arasında sağlık çalışanların ve toplumun bilgilendirilmesine yönelik bilimsel ve güncel bilgilerin paylaşılması; hemşirelerin bu süreçte yaşadıkları sorunlar ve bu sorunlara yönelik çözüm önerilerin rapor hâline getirilerek Sağlık Bakanlığı ve ilgili birimlere iletilmesi; bakım algoritmaları ve kişisel koruyucu ekipman kullanımı videolarının hazırlanıp paylaşılması yer almaktadır. Ülkemizde hemşireler COVID-19 pandemi sürecinde, güvenli ve nitelikli hemşirelik bakımını en üst düzeyde sürdürmek için zor şartlar altında mesleklerini en iyi şekilde icra etmiştir. Bununla birlikte, hemşirelerin politikaları oluşturma ve karar alma süreçlerinde etkili rol alması; liderlik pozisyonlarında daha fazla hemşirelere yer verilmesi için gerekli girişimlerin ve yatırımların ivedilikle yapılması gerekmektedir.Publication Metadata only Knowledge and practices of operating room nurses in the prevention of pressure injuries(Elsevier Sci Ltd, 2022) Cebeci, Fatma; Çelik, Sevilay Şenol; Faculty Member; School of Nursing; 5676Background: Operating room (OR) nurses play an important role in preventing the pressure injuries (PIs) that may develop during the perioperative process. This study was conducted to determine OR nurses' level of knowledge about PIs, and how they manage them. Methods: This is a cross-sectional, quantitative and descriptive study. The sample of the study consisted of 234 OR nurses working in eight different public hospitals in Ankara. The questionnaire applied in this study was prepared in accordance with the guidelines. This questionnaire consisted of three sections: demographic profiling, common preventive practices, and the knowledge of OR nurses about intraoperative PI prevention. Findings: 66.7% of the participants had received education about PIs during their basic nursing training, and 41.5% had received education after graduation. 97.4% of OR nurses did not follow international guidelines about PIs. The mean total score of the OR nurses for the questions about PIs was 52.0 +/- 13.7 out of a possible score of 100. The lowest mean score was obtained for the topic of 'staging pressure injuries', and the highest score was obtained from 'interventions to prevent pressure injuries'. In addition, 81.5% of the OR nurses stated that they were not given information about patients with a high PI risk by clinical nurses. 97.9% of the OR nurses did not use a scale to assess intraoperative PI risks. Fewer than half of the nurses said that they assessed the risk of PIs during surgery. 90.8% the nurses did not record risk assessment and interventions to prevent PIs. Conclusion: There is a need to incorporate basic operating room PI (ORPI) training into both the basic nursing and in-service training to improve the knowledge of OR nurses about how to prevent and manage ORPIs. The aim of the study was to determine feelings, thoughts, and attitudes of nursing students regarding the care of dying patients. Material and Methods: A partially mixed concurrent dominant status design was used in the study. Focus group interviews were conducted to determine nursing students’ feelings about and views of the care of dying patients. Five focus group interviews were conducted at the qualitative data collection stage. Frommelt Attitudes Toward Care of the Dying Scale was used to measure attitudes. Results: Four themes were identified in the qualitative data analysis: (1) feelings experienced while caring dying patients; (2) difficulties encountered while providing care to dying patients; (3) strategies of coping with the difficulties and (4) reasons for feeling incompetent in the care of dying patients. Nursing students reported they experienced intense sadness, desperation, and incompetence in the care of dying patients. The most common reasons for feeling incompetent were lack of experience, knowledge, and skill. Nursing students’ mean total score on the Frommelt Attitudes Toward Care of the Dying Scale was 92.83±18.15. Conclusion: Nursing students were seen to have positive approach in the caring for dying patients, although they experienced difficulties and were often unable to cope with the situation. Nursing education curriculum for the care of dying patients should be included current training methods such as simulation and case-based learning to improve experience, confidence and competence of nursing students. /Öz: Amaç: Hemşirelik öğrencilerinin ölmekte olan hastaların karmaşık bakımına katılmaya istekli olmadığı düşünülmektedir. Bu çalışma hemşirelik öğrencilerinin ölmekte olan hastaya bakım konusunda duygu, düşünce ve tutumlarını belirlemek amacıyla yapılmıştır. Gereç ve Yöntemler: Çalışmada kısmen karma eşzamanlı baskın statülü tasarım karma araştırma yöntemi kullanılmıştır. Hemşirelik öğrencilerinin ölmekte olan hastanın bakımına yönelik duygu ve düşünceleri odak grup görüşmeleri ile elde edilmiştir. Nitel verilerin toplanmasında beş odak grup görüşmesi gerçekleştirilmiştir. Tutumları belirlemek için Frommelt Ölmekte Olan Bireye Bakım Vermeye İlişkin Tutum Ölçeği kullanılmıştır. Bulgular: Çalışmada nitel verilerin analizi aşamasında dört tema belirlenmiştir: (1) ölmekte olan hastalara bakım verirken deneyimlenen duygular; (2) ölmekte olan hastalara bakım verirken karşılaşılan güçlükler; (3) güçlüklerle başa çıkma stratejileri; (4) ölmekte olan hastaya bakım verirken yetersiz hissetme nedenleri. Hemşirelik öğrencileri, ölmekte olan hastalara bakım verirken yoğun üzüntü, çaresizlik ve yetersizlik deneyimlediklerini ifade etmiştir. Yetersizlik duygusunun en önemli nedenleri deneyim, bilgi ve beceri eksikliği olarak belirlenmiştir. Hemşirelik öğrencilerinin Frommelt Ölmekte Olan Bireye Bakım Vermeye İlişkin Tutum Ölçeği puan ortalaması 92.83±18.15’dir. Sonuç: Hemşirelik öğrencilerinin, deneyimledikleri zorluklara ve çoğu zaman bu durumla başa çıkamamalarına rağmen ölmekte olan hastaya bakım vermek konusunda olumlu bir tutum içerisinde oldukları belirlenmiştir. Cardiovascular surgery nurses, whoare experienced in critical patient care, mechanical ventilation support,cardiopulmonary resuscitation, mechanical circulation support, andemergency situation management, also served on the pandemic teams.In this article, the duties and responsibilities of cardiovascular surgerynurses, their problems and needs during the pandemic process, andtheir contribution to the process were evaluated./Öz: Çin’de başlayarak tüm dünyayı etkisi altına alan yeni tip koronavirüs hastalığı, yüksek yayılım hızı ve mortalite oranları nedeniyle Dünya Sağlık Örgütü tarafından pandemi olarak ilan edilmiştir. Hastalığın hızlı yayılımı ve yoğun bakım gereksinimi, tüm dünyada ve Türkiye’de pandemi hastanelerinin oluşturulmasına neden olmuştur. Pandemi hastanelerinde koronavirüs olgularının yönetiminde görev almak üzere özel ekipler oluşturulmuştur. Bu ekiplerin en çok sorumluluk alan en önemli üyelerinden biri hemşirelerdir. Farklı alanlarda çalışan hemşireler bu süreçte pandemi klinik ve yoğun bakımlarında görev almışlardır. Kritik hasta bakımı, mekanik ventilasyon desteği, kardiyopulmoner resüsitasyon, mekanik dolaşım desteği, acil durum yönetimi konularında deneyimli olan kalp damar cerrahi hemşireleri de pandemi ekipleri içerisinde hizmet vermiştir. Materials and methods: This interventional study was carried out in a university hospital with a bed capacity of 1114 in an urban center in Turkey. Ethics committee approval (28.06.2018/31) and institutional permission were obtained for the study, in addition to the nurses' written, informed consent. The study was completed in three stages. In the first stage an initial PP study was conducted in the clinics with the participation of the nurses and the members of the research team (n = 422 patients). In the second stage the knowledge levels of 194 nurses were measured before training was given on following-up and preventing PIs. The nurses then participated in the tailored training and their knowledge levels were re-measured afterwards. All the nurses were given individual advice related to the prevention of PIs for 30 days after they had completed the training. In the third stage a second PP study was conducted four months after the first PP study (n = 454 patients). The data were collected using the Pressure Injury Prevalence Form, the Braden Pressure Ulcer Risk Assessment Tool and the Knowledge Level Measurement Form. Descriptive values, the paired samples t-test, Pearson's chi-squared test and Fisher's Exact test were used to evaluate the data. Results: The nurses' pretest mean knowledge score was 55.36% +/- 14.40 and their posttest mean score was 69.92% +/- 9.73. The difference between these scores was statistically significant (p < 0.05). The study found no significant difference between the first PP ratio and the second PP ratio (p > 0.05), and the nurses were better able to evaluate skin and PIs after the training. Conclusion: The study determined that the tailored training given to the nurses increased their knowledge; however, it had no impact on the PP after four months. Nurses play a vital role in both care in intensive care units/clinics and post-discharge process management. This study focuses on the current literature to discuss the main characteristics of ECMO and the nursing care provided to patients on ECMO. / Öz: Dünyada ve Türkiye’de ekstrakorporeal membran oksijenizasyonu (ECMO) uygulama sıklığının ve uygulama sonrası taburculuk oranlarının giderek artması, ECMO uygulanan hastaların özellikli bakımına dikkat çekmektedir. ECMO uygulanan hastalar multidisipliner bir ekip tarafından verilen özel bir bakım gerektirmektedir. Hemşireler bu ekip içerisinde, hem yoğun bakım üniteleri ve kliniklerdeki bakımın hem de taburculuk sonrası sürecin yönetiminde önemli sorumluluklar üstlenmektedir. This study systematically reviewed and analysed the qualitative studies concerning perioperative nurses' experiences of robotic-assisted surgery. This systematic literature review included studies up to December 2020. The study data were analysed using inductive content analysis. This systematic review included six articles. There were a total of 71 nurses who participated in the included articles (min = 6, max = 17). Their mean age was 35.7, and their experience in robotic-assisted surgery ranged from 8 months to 10 years. Content analysis generated six categories: adaptation to robotic-assisted surgery technology, the importance of teamwork in robotic-assisted surgery, changing tasks and responsibilities in robotic-assisted surgery, training requirements for robotic-assisted surgery, the effects of robotic-assisted surgery on patients and patient safety, and difficulties with robotic-assisted surgery. A variety of themes and sub-themes emerged in these categories. The review highlights the importance of developing new ways of thinking about the assessment and management of disruptions, developing different teamwork patterns and communication skills, and overcoming the challenges involved in technologically advanced surgeries. Nurses' roles in robotic technology should be redefined in healthcare. Nurses should learn how to adapt to advancing technology and how to supplement and enhance their skills.Publication Metadata only Effect of protective cover on the quality of life of patients with tracheostomy-a quasi-experimental study(Elsevier, 2020) Seyman, Cigdem Canbolat; N/A; Çelik, Sevilay Şenol; Faculty Member; School of Nursing; 5676Purpose: Patients with tracheostomy experience problems such as the inability to speak, swallowing disorders, and impairment of body image. These problems adversely affect patients' quality of life (QoL). This study aimed to investigate the effect of discharge training and the use of a protective cover on the QoL of patients with tracheostomy and the problems experienced after discharge. Design: A quasi-experimental study. Methods: All patients (n = 24) were followed for 3 months after the preoperative period. Patients in the control group (CG) received routine care after surgery, whereas patients in the intervention group (IG) received routine care, discharge training, and a protective stoma cover. Findings: The 90th postdischarge day QoL scores between the CG and IG were statistically significant. Respiratory problems were the most common postdischarge problems in both groups. Patients in the CG significantly experienced more problems than those in the IG during the postdischarge period. Conclusions: This study provides that patients can benefit from discharge training and protective cover and that health care professionals become aware of the cover for future use. Results of studies comparing the effects of preoperative skin preparations are contradictory. PURPOSE: This study aimed to determine the effect of different preoperative skin preparation methods on the rate of SSIs in patients undergoing sternotomy. METHODS: A quasi-experimental study was conducted among 96 male patients undergoing sternotomy. The control group (CG) (n = 34) received routine care consisting of shaving body hair with a razor blade followed by instructions to take a bath or shower. In the intervention groups, patients received education about SSI prevention and body hair was removed with an electric clipper, followed by bathing with daphne soap containing olive oil (16-1) (n = 31) or 2% chlorhexidine solution (IG-2) (n = 31). Patient demographic, medical history, surgical, and wound assessment variables were obtained. Potential 551 signs and symptoms were assessed for up to 90 days following surgery. RESULTS: Patient demographic, medical history, and surgical variables did not differ among the 3 groups. Sternal SSI occurred in 10.4% of all study patients; 8.8% of the CG patients, 12.9% of the IG-1 patients, and 9.7% of the IG-2 patients developed an 551 (P> .05). CONCLUSION: There were no significant differences in the rate of sternotomy SSI among the 3 groups. Randomized controlled trials with large samples are needed to compare these methods to determine optimal and affordable preoperative skin preparation methods.
