A comprehensive MRI-based study on adhesive capsulitis and its correlations with tendinosis and systemic diseases

dc.description.abstractThe association between adhesive capsulitis and hypothyroidism is well-known. However, its relationship with hypertension and diabetes remains debated. While, rotator cuff (RC) tendinosis is frequently observed, to our knowledge, there is no study indicating its link to adhesive capsulitis. This study aimed to assess the prevalence RC tendinosis in adhesive capsulitis and examine associations between adhesive capsulitis and hypertension, diabetes, thyroid dysfunction, and inferior glenohumeral ligament (IGHL) thickness. METHODS: This retrospective study included 503 patients with adhesive capsulitis who underwent magnetic resonance imaging (MRI) of the shoulder. Patients with previous shoulder trauma, surgery, or rheumatologic disease were excluded. Clinical data on hypertension, diabetes, and thyroid dysfunction were collected. The IGHL thickness was measured, and RC tendinosis was classified according to tendon involvement. Statistical analyses included t tests, chi-square tests, logistic regression, and receiver operating characteristic analysis. RESULTS: Rotator cuff tendinosis was observed in 76.94% of patients, with supraspinatus tendinosis being the most common (74.95%). Thyroid dysfunction was significantly more prevalent in patients with adhesive capsulitis (15.90%, p < 0.001), while hypertension was lower (21.87%, p < 0.001) compared to the general population. The prevalence of diabetes (12.52%) was not significantly different from the general population (p = 0.426). Diabetes was associated with increased IGHL thickness (p = 0.005), whereas thyroid dysfunction was linked to lower IGHL thickness (p = 0.023). The IGHL thickness did not predict tendinosis (AUC = 0.47). CONCLUSION: Supraspinatus tendinosis is overrepresented in patients with adhesive capsulitis, with a prevalence of 75%. Thyroid dysfunction may contribute to its pathogenesis as indicated in previous research. Additionally, IGHL thickness alone is not a reliable predictor of tendinosis. Further studies are needed to explore these associations. ZUSAMMENFASSUNG: HINTERGRUND: Der Zusammenhang zwischen adhäsiver Kapsulitis und Hypothyroidismus ist wohl bekannt. Weiterhin diskutiert wird jedoch der Zusammenhang zwischen adhäsiver Kapsulitis und Hypertonie sowie Diabetes. Eine Tendinose der Rotatorenmanschette wird zwar häufig festgestellt, aber nach Wissen der Autoren liegt bisher keine Studie mit Hinweis auf einen Zusammenhang dieser mit adhäsiver Kapsulitis vor. Ziel der vorliegenden Studie war es, die Prävalenz der Tendinose der Rotatorenmanschette bei adhäsiver Kapsulitis und den Zusammenhang zwischen adhäsiver Kapsulitis und Hypertonie, Diabetes, Schilddrüsenfunktionsstörung sowie Dicke des Ligamentum glenohumerale inferius (IGHL) zu untersuchen. METHODEN: In diese retrospektive Studie wurden 503 Patienten mit adhäsiver Kapsulitis einbezogen, bei denen eine Magnetresonanztomographie (MRT) der Schulter durchgeführt wurde. Patienten mit Zustand nach Schultertrauma, -operation oder rheumatologischer Erkrankung wurden ausgeschlossen. Es wurden klinische Daten zu Hypertonie, Diabetes und Schilddrüsenfunktionsstörungen erhoben. Die Dicke des IGHL wurde gemessen und die Tendinose der Rotatorenmanschette gemäß der Sehnenbeteiligung klassifiziert. Zu den statistischen Analysen gehörten t-Tests, Chi-Quadrat-Tests, logistische Regressions- und Receiver-Operating-Characteristic-Analysen. ERGEBNISSE: Eine Tendinose der Rotatorenmanschette wurde bei 76,94% der Patienten diagnostiziert, dabei war die Supraspinatustendinose die häufigste Form (74,95%). Bei Patienten mit adhäsiver Kapsulitis bestand eine signifikant höhere Prävalenz für Schilddrüsenfunktionsstörungen (15,90%; p < 0,001), während die Prävalenz für Hypertonie niedriger war (21,87%; p < 0,001) als bei der Allgemeinbevölkerung. Die Prävalenz von Diabetes (12,52%) unterschied sich nicht signifikant von der bei der Allgemeinbevölkerung (p = 0,426). Diabetes stand im Zusammenhang mit einer erhöhten Dicke des IGHL (p = 0,005), während Schilddrüsenfunktionsstörungen mit einer geringeren Dicke des IGHL vergesellschaftet waren (p = 0,023). Die Dicke des IGHL stellte keinen Prädiktor für Tendinose dar (Fläche unter der Kurve, AUC: 0,47). SCHLUSSFOLGERUNG: Die Supraspinatustendinose war bei Patienten mit adhäsiver Kapsulitis mit einer Prävalenz von 75% überrepräsentiert. Schilddrüsenfunktionsstörungen tragen früheren Studie zufolge möglicherweise zur ihrer Pathogenese bei. Außerdem ist die Dicke des IGHL allein kein zuverlässiger Prädiktor für Tendinose. Weitere Studien sind notwendig, um diese Zusammenhänge zu untersuchen.
