Abstract
Ideal treatment outcomes for vocal fold leukoplakia include the complete removal of diseased epithelium with minimal vocal fold scarring and preservation of vocal quality. Aminolevulinic acid (ALA) Photodynamic Therapy (PDT) is a promising novel technique described in this chapter that may better realize these ideal treatment outcomes. After topical application of ALA, the porphyrin-heme biosynthesis pathway converts the ALA, leading to the accumulation of protoporphyrin IX, a photosensitizing agent, in the mitochondria. A laser is used to precisely activate the protoporphyrin IX to target its lethal cellular effects in the diseased tissue. Over the past 24 years, ALA-PDT has been a safe and effective technique for the treatment of glottal dysplasia.
