E.; Sanli, I.; N/A; Taşkıran, Ali Sarper; Other; School of Medicine; 195168Objective: Factors contribute to commercial sexual exploitation of youth (CSEY) and a variety of physical and psychological consequences of CSEY were previously discussed in the literature. The aim of the current study was to determine the long term prognosis of a sample of CSEY in Turkey. Methods: A sample of 108 CSEY (all females) with a mean age of 16.7 +/- 2.7 (range: 9-22) years living in a specialized center for protection against perpetrators were examined in terms of their sociodemographic variables, previous abuse history, family history and current psychopathology. Identified cases were referred for psychiatric evaluation. We obtained follow-up data 11 years after initial contact from this cohort (mean age 25.9 +/- 2.6 at time of follow-up) with the subjects and received information regarding their current functioning. Results: Follow up data revealed that recommendations during psychiatric follow up could not be acted upon. 71.4% of the initial sample (n= 76) could be reached via direct or indirect contacts. We found that good/superior functioning was very rare and that one-third of the sample that could be reached still was being sexually exploited. Important considerations derived from the results were discussed in the current article.Publication Open Access Effects of bariatric surgery on psychological well-being among adolescents: A case series study from Turkey(Anatolian Journal of Psychiatry Group, 2020) Çöpür, Mazlum; Saatçioğlu Tinkir, Nilüfer; Çöpür, Sidar; Researcher; School of MedicineObjective: our aim is to determine the psychiatric outcomes of bariatric surgery among adolescent patients with obesity, a condition with high psychiatric comorbidity rates. Methods: psychiatric evaluation of the patients includes Beck Depression Inventory, Beck Anxiety Inventory, Eating Attitudes Test and World Health Organization Quality of Life questionnaire (WHOQOL-BREF) prior to surgery and at post-operative year. Results: adolescent patients admitting for bariatric surgery have high rates of psychiatric comorbidities. We report significant improvement in psychological well-being of the adolescent patients following sleeve gastrectomy procedure including depression, anxiety and feeding behavior scores and quality of life. Discussion: limited number of studies regarding the psychiatric outcomes of bariatric surgery on adolescent patients are present in the literature. Improve-ment in depression and quality of life scores have also been reported in previous studies, whereas, no comprehensive study on anxiety and feeding behavior scores, to our knowledge, had been performed. Therefore, our case series study is significant by demonstrating the psychiatric outcomes of bariatric surgery more thoroughly. In addition, higher reductions in BMI have been observed following sleeve gastrectomy procedure compared to the findings in the literature mostly including roux-en-Y gastric bypass procedure in adults. We propose that psychiatric comorbidities of the adolescent patients should be more prominent determinant of decision making procedure for bariatric surgery. / Amaç: bu çalışmada ergenlik çağındaki obezite hastalarında bariyatrik cerrahinin psikiyatrik sonuçlarını belirlemeyi amaçladık. Yöntem: hastaların psikiyatrik değerlendirme süreci Beck Depresyon Ölçeği, Beck Anksiyete Ölçeği, Yeme Tutumu Testi ve Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeğini (WHOQOL-BREF) içermektedir. Değerlendirme ameliyat öncesi dönemde ve ameliyat sonrasında 1. yıl takibinde yapılmıştır. Sonuçlar: bariyatrik cerrahi amacıyla kliniğe başvuran ergenlik dönemi hastalarında eşlik eden psikiyatrik bozukluklar sık görülmektedir. Çalışmamızın sonucunda tüp mide ameliyatı sonrasında hastaların ruhsal durumlarında depresyon, anksiyete, yeme tutumu ve yaşam kalitesi ölçekleri de dahil olmak üzere önemli oranda iyileşme olduğunu gözledik. Tartışma: bariyatrik cerrahinin psikiyatrik sonuçlarıyla ilişkili alan yazında az sayıda çalışma vardır. Depresyon ve yaşam kalitesi ölçeklerinde gözlemlediğimiz iyileşmeler önceki çalışmalarla uyumlu olup hastaların anksiyete ve yeme tutumlarındaki değişimleri inceleyen kapsamlı bir çalışma yoktur. Çalışmamız ergenlik döneminde bariyatrik cerrahi müdahale geçiren hastaların psikiyatrik durumlarını daha ayrıntılı incelemesi bakımından önemlidir. Ayrıca çalışmamızda tüp mide ameliyatı sonucunda ergenlik dönemi hastalarımızda beden kitle indeksinde alan yazında çoğunlukla yetişkin hastalarda yapılan roux-en-Y gastrik baypass ameliyatı sonucunda bildirilenden daha yüksek miktarda bir iyileşme görmekteyiz. Ergenlik dönemi hastalarında hastaların eşlik eden psikiyatrik bozukluklarının bariyatrik cerrahi kararı alınırken, kararın daha önemli bir parçası olmasını önermekteyiz.Publication Open Access Three adolescent cases of a very rare disorder: trichotemnomania(Taylor _ Francis, 2019) Ateş, Bilge; Nasıroğlu, Serhat; Eray, Şafak; N/A; Mutluer, Tuba; Faculty Member; School of Medicine; 305311Trichotemnomania (TT) is characterized as the cutting or shaving of hair, which is an obsessive-compulsive habit. TT takes its name from a fusion of Greek words: thrix (hair), temnein (to cut), and mania (madness). TT is a very rare disease with only five case studies reported in the literature and to the best of our knowledge; no data are available on adolescents. This study focuses on three cases of adolescents with TT. All patients have been evaluated by a dermatologist and two child and adolescent psychiatrists. Dermatological examinations included medical history, physical examination, dermoscopy assessment, and laboratory investigations. Detailed psychiatric assessments consisted of socio-demographic data, clinical history, semi-structured interviews, and psychometric tests. Three cases with TT were both diagnosed with obsessive-compulsive disorder. Two of them diagnosed comorbid social anxiety disorder (in cases 1 and 2), and one case diagnosed with comorbid general anxiety disorder (in case 3) and agoraphobia (in case 3). Cases 1 and 2 were prescribed 50 mg/day of sertraline; however, case 3 refused the treatment. In conclusion, TT may be confused with trichotillomania or other disorders presented with alopecia. Differential diagnoses may be due to histopathological changes or dermoscopic assessment. Dermatologists should consider TT when a supposed alopecia areata looks somewhat unusual and should refer these patients for psychiatric evaluation.Publication Open Access Can visual hallucinations be among the neuropsychological manifestations of systemic mastocytosis?: a geriatric case(Aves, 2020) Çetiner, Mustafa; Kılıç, Özge; Ercan, Alaattin Cenk; Komesli, Zeynep; Yılmaz, Uğur Eser; Doctor; School of Medicine; Koç University HospitalMast cells are considered sensors of environmental and emotional stress, exist in all body parts and are related to the pathway from stress to inflammation. Mastocytosis defines a rare disease characterized by the accumulation of abnormal mast cells in multiple organs. Here, we present a 77-year-old woman with a background of aggressive systemic mastocytosis who developed impaired cognition, depression, anxiety, visual hallucinations, delusions, and insomnia. Symptoms alleviated only after initiating midostaurin, a tyrosine kinase inhibitor. We also analyzed the features of sleep disorders in patients with epilepsy to demonstrate the effect of seizures and seizure types on sleep. Methods: our study assessed 43 patients with epilepsy and 53 age- and gender-matched healthy controls. The demographic and clinical data of all participants were recorded. The Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index (PSQI), International Restless Legs Syndrome Study Group Rating Scale, Berlin Questionnaire, and Beck Depression Inventory (BDI) were administered to all study subjects. The interview used to evaluate insomnia is based on the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition - DSM-5 diagnostic criteria. Results: twenty-four patients (55.8%) and 26 controls (49.1%) are women. The mean age of patients and controls was 34.2 +/- 11.37 (16-71) and 34.6 +/- 11.28 (16-77), respectively. Patients with epilepsy had depression more often than controls, a result that was statistically significant (p<0.0001). We found no statistically significant difference between sleep parameters of patients and controls with normal BDI scores (p>0.05). Patients with depression had worse results on the Berlin Questionnaire and PSQI total score, with statistical significance (p=0.002). Nocturnal seizures, seizure type, and drug treatment had no effect on sleep (p>0.05). Conclusion: we concluded that depression rather than epilepsy negatively affects sleep, suggesting that all patients should be asked about their mood and sleep complaints.Publication Open Access Depression, but not dissociative experiences, predicts overgeneral memory: a systematic review and meta-regression analysis(Aves, 2022) Okan, A.; Erkent, M.A.; Department of Psychology; Aydın, Fatma; Şar, Vedat; Gülgöz, Sami; Eser, Hale Yapıcı; Faculty Member; Faculty Member; Faculty Member; Department of Psychology; College of Social Sciences and Humanities; School of Medicine; N/A; 8542; 49200; 134359Background: reduced memory specificity (i.e., overgeneral memory) is a characteristic of autobiographical memories widely studied in clinical populations, and it is explained by rumination, functional avoidance, and executive dysfunction. Though the relationship of autobiographical memory specificity with mood and anxiety disorders has been shown, how it relates to dissociation is not well-established. Thus, we aimed to investigate whether dissociative experiences are related to overgeneral memory while considering concurrent depression as a possible confounding factor. Methods: we conducted a systematic review in compliance with The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines and searched PubMed and Web of Science databases using autobiograph* and dissoc* as our keywords. Results: of the 768 studies identified, 9 studies fulfilled the inclusion criteria. A meta-regression analysis was conducted to analyze the relationship between dissociative experiences and depression scores with autobiographical memory test scores. Our research revealed that depression scores, but not dissociative experiences, are significantly related to reduced memory specificity. Conclusion: while the possible overlap between dissociation and depression should be considered in the interpretation of the findings, dissociative experiences do not seem to pose vulnerability for reduced specificity of autobiographical memory. The number of studies on the topic is limited, and they do not have longitudinal follow-ups.
