However, systematic lymphadenectomy may cause significant morbidity. In recent years, the use of sentinel lymph node (SLN) mapping with indocyanine green (ICG) has been accepted to avoid the morbidity of lymphadenectomy. We aimed to assess the diagnostic accuracy of a novel injection technique in detection of sentinel lymph nodes in women with endometrial cancer. Methods: A total of 214 patients with endometrial cancer underwent sentinel lymph node mapping using ICG. ICG was injected into the uterine cervix at the 3 and 9 o’clock positions, submucosally and to the level of junction between uterine cervix and isthmus in group 1(n=107) and to the uterine cervix at the 3 and 9 o’clock positions according to conventional Memorial Sloan Kettering algorithm in group 2(n=107). All the patients in group 2 selected by propensity matching. None of the patients underwent a re-injection neither in group 1 nor group 2. Results: There was no significant difference between baseline characteristics of two groups. The groups were similar in terms of stage, type of tumor, BMI and lymphovascular space invasion. The bilateral detection rates were 94.4% and 76.6% in group 1 and group 2, respectively (p=0.003). No lymph node or lymphatic vessels were identified in only one patient with a history of chronic lymphocytic leukemia in group 1. 193687Purpose: As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. Patients and Methods: This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). Results: Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). Conclusion: Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks. The databases of seven gynaecologic cancer centres were searched in order to identify all consecutive gynaecologic cancer patients undergoing major surgery between March 11, 2020 and May 15, 2020 for this retrospective, case-control study. The case group consisted of patients with histopathologically confirmed gynaecologic cancers, and each case was matched with two counterparts who had undergone primary surgery before the COVID-19 pandemic. The case and the control groups were compared in terms of length of hospital stay, admission to the intensive care unit (ICU), intraoperative and postoperative complications. During the study period, 154 women with gynaecologic cancer undergoing major surgery were identified. Although the case group had more co-morbidities compared to the control group (103/154 vs. 178/308, respectively; p = .04), the median length of hospital stays, the rate of ICU admission, intraoperative complication rates and postoperative complication rates were similar in the two groups. Gynaecologic cancer surgery may be performed safely in the COVID-19 era with similar rates of ICU admission, intraoperative and postoperative complications compared to the patients operated before the COVID-19 pandemic.IMPACT STATEMENT What is already known on this subject? Many societies have announced their guidelines about the surgical management of gynaecologic cancer patients during the COVID-19 pandemic. However, most of them are not evidence-based and mostly on expert opinions. What do the results of this study add? The main findings of this retrospective, case-control study indicate that the short-term (30 day) outcomes of gynaecologic cancer patients undergoing major surgery in the COVID-19 era are similar to those who had been operated before the COVID-19 pandemic. The length of hospital stays, the rates of admission to the ICU, intraoperative and postoperative complications were comparable between women undergoing major gynaecologic cancer surgery in the COVID-19 era and the women who had been operated before the pandemic. What are the implications of these findings for clinical practice and/or further research? As a result, providing treatment options for fertility preservation in women with gynecologic cancer has become an important component of cancer survivorship care. Loss of fertility is considered one of the most significant effects of cancer treatment and this concern is an additional stress factor among women of reproductive age. Modern chemotherapy drugs and radiotherapy regimens have a substantial negative effect on reproduction. Chemotherapy drugs exert cytotoxic effects systematically and so that, can damage the ovaries, leading to infertility and premature ovarian failure. Radiation is harmful to both the ovaries and uterus, as a result of that causing a greater magnitude of adverse effect on the female reproductive function. We aimed to assess the level of information about COVID and the depression rate among the healthcare workers. Our study was conducted on 89 healthcare workers. This survey was held among healthcare workers who worked in COVID-19 service during the pandemic in Turkey. As a guideline to adequately identify depression rates we used Beck Depression Inventory. Each question was scored in the Beck depression questionnaire. The service period length of the healthcare personnel participating in the study were 0-2 years in 34 (38.2%), 2-5 years in 25 (28.1%), 5-10 years in 12 (13.5%), and >10 years in 18 (20.2%) healthcare workers. There was a statistically significant difference between the service period of healthcare workers and their COVID-19 knowledge scores (p<0.05). Thirty (33.7%) of 89 healthcare workers had minimal depression, 30 (33.7%) had mild depression, 19 (21.3%) had moderate depression, and 10 (11.2%) had severe depression. There was no statistically significant difference between the knowledge levels of healthcare professionals and their depression scores (p>0.05). The survey results about COVID-19 highlighted that psychological support should be ensured, adequate education/training should be provided. Additionally, psychosocial needs should be determined, and psychosocial services should be given to healthcare workers. © 2022, Yuzuncu Yil Universitesi Tip Fakultesi. Human papillomavirus (HPV) infection is a risk factor for cervical carcinoma. Over 100 types of HPV have been identified. The excisional procedures are recommended for women with high-grade cervical intraepithelial neoplasia. Surgical margin status is an important predictor of the risk of relapse. The aim of the current study was to evaluate whether HPV genotype is a predictive factor of positive surgical margin after cervical cone excision. Materials and Methods. The records of 448 HPV-infected patients who underwent loop electrosurgical excision or cold knife conization at a tertiary gynecological cancer center were retrospectively reviewed. The patients were divided into 6 groups according to HPV positivity: HPV 16 only, HPV 18 only, HPV 16/18, other high-risk HPV (hrHPV), HPV 16/hrHPV, and HPV 18/hrHPV. Results. There was no significant difference between the HPV groups in terms of age, parity, menopausal status, endocervical canal involvement, conization method, and the rates of positive margin (P = .15, P = .49, P = .07, P = .20, P = .24, P = .39, respectively). Conclusion. The results show that HPV subtypes might not be associated with endocervical canal involvement and the rates of positive margin. Here, we present a case of postmenopausal ovarian high-grade serous carcinoma associated with ovarian endometrioma. A 64-year-old woman attended the clinic with pelvic pain and persistent vaginal discharge. Ultrasonography revealed an endometrial thickness and 37*23 mm left cystic adnexal mass. During the operation, solid pelvic mass grossly located in the cul-de-sac infiltrating rectum and left ovarian cysts were observed. At the intraoperative frozen section, benign left adnexal mass, benign pelvic lymph nodes, and the morphological findings of adenocarcinoma with undetermined-site of the origin were detected in the uterus and rectosigmoid colon. Final pathology revealed a high-grade ovarian serous carcinoma arising in ovarian endometrioma from two microscopic focuses. In sum, when we encounter postmenopausal endometrioma with increased biomarkers, it is crucial to consider the possibility of malignant transformation to ovarian cancer. / Öz: Postmenopozal ovarian endometriomadan kaynaklanan yüksek dereceli seröz over kanseri vakasını sunmayı amaçladık. 64 yaş kadın hasta pelvik ağrı ve vajinal akıntı şikayeti ile kliniğimize başvurdu. Ultrason görüntülemesinde endometrial kalınlaşma ve sol adneksial alanda 37*23 mm kistik kitle saptandı. Operasyon sırasında solid pelvik kitlenin cul-de-sac yerleşimli ve rektum ile sol overi infiltre ettiği görüldü. Frozen incelemede benign sol adneksial kitle, benign lenf nodları ve rektosigmoid kolon ile uterusta kökeni belirsiz adenokarsinoma ait morfolojik bulgular saptandı. Nihai patoloji sonucu iki mikroskopik odakta ovarian endometriomadan kaynaklı yüksek dereceli ovarian seröz kanser olarak geldi. Endometriomadan kaynaklanan epitelyal seröz over kanseri az görülen bir durumdur. Most ovarian cancer patients are diagnosed at an advanced stage, and more than half of them have upper abdominal involvement. Upper abdominal regions alongside the pelvis should be evaluated systematically as a routine procedure during cytoreductive surgery. Therefore, aggressive procedures are adopted during cytoreductive surgery, including upper abdominal regions, to achieve maximal cytoreduction. It should include the exploration of porta hepatis and celiac lymph nodes. The feasibility of metastatic disease resection at the porta hepatis and celiac lymph nodes has been demonstrated in many studies with acceptable morbidity. Furthermore, ovarian cancer often leads to retroperitoneal lymph nodes metastases in patients with advanced stages of the disease. Data from the literature showed that more than half of the advanced-stage ovarian cancer patients had lymph node involvement. In this manuscript, we reviewed the current literature and aimed to investigate the impact on survival of surgical resection of porta hepatis, celiac regions, and pelvic/paraaortic lymph nodes in patients with advanced-stage ovarian cancer. Resection of metastatic disease at the porta hepatis/celiac lymph nodes to achieve maximal cytoreduction is feasible but with a relatively high rate of morbidity and mortality. Randomized controlled trials indicate that in the absence of suspicious lymph nodes, both during surgery and at imaging, systematic lymphadenectomy seems to provide no survival benefit.Publication Open Access Factors affecting the histopathological outcomes of atypical glandular cells on pap test(Medknow Publications, 2021) Keleş, Esra; Öztürk, Uğur; Alınca, Cihat; Kabaca, Canan; Çetiner, Handan; Giray, Burak; Faculty Member; Koç University HospitalBackground: glandular cell abnormalities may indicate the presence of pre-malignant or malignant lesions. Aim: this study aimed to investigate the relationship between atypical glandular cells (AGC) and patients' demographics, histopathological outcomes, Human Papillomavirus (HPV) test results. Material and methods: between January 2015 and December 2019, women with AGC on Pap tests were retrieved from the hospital electronic database. The patients with AGC on cervicovaginal smears who underwent further pathological, laboratory, and imaging diagnostic testing and who were followed up at least 1-year were included in the study, while those who had a history of cervical dysplasia or cancer, lost during follow-up, or had missing data were excluded. Results: of 85,692 Pap smears, 114 (0.13%) were diagnosed with AGC, of those 88 cases were eligible for final analysis. Gynecological malignancies were detected in 13 (14.8%) patients; including 6 (6.8%) endometrioid endometrial cancers, 3 (3.4%) non-endometrioid endometrial cancers, 2 (2.3%) cervical adenocarcinomas, 1 (1.1%) cervical squamous cell carcinoma, and 1 (1.1%) high-grade tubal serous cancer. Multivariate analysis revealed that presence of concomitant abnormal squamous lesion (P = 0.002), being 50 years and older (P = 0.028), HPV positivity (P < 0.001), and menopause (P = 0.023) were risk factors for significant pathology. Conclusion: the diagnosis of AGC may be related to the preneoplastic/neoplastic processes. A further comprehensive histopathological examination is required in women with AGC, aged 50 years and older, postmenopausal, HPV-positivity and concomitant squamous cell abnormality Clinicians should consider ovarian pathologies when there is no pathological finding on endometrial or cervical histopathological examination.
